Diabetes Prevention Program:

Comunidad Saludables de la Montana (CSDLM) offers the ADA's Diabetes Prevention Express Program (DPP) through a bilingual distance learning and telehealth infrastructure.

This program provides specialized clinical services for managing chronic conditions, connected to remote devices, to help Medicare-insured older adults, private patients in Puerto Rico, and communities throughout the United States manage their conditions.

CSDLM also has a network of diabetes prevention services and lifestyle advisors certified by the University of Puerto Rico, ensuring the necessary support to reverse prediabetes and promote healthy lifestyles from anywhere.
